This new form is to be completed by referral agency only. Here are instructions for this process:

  1. Select the appropriate gender and age from the list on the menu on the right side of this
  2. Complete one form for each individual – only one person can be scheduled per form.
  3. Select the date and time for each appointment. If scheduling multiple members of a
    family for the same time and there are not enough simultaneous time slots available,
    select the closest empty time slot available so families can arrive together.
  4. Please note that we do not request client emails, phone numbers, or addresses any longer.
  5. Please note that we now have drop-down menus for some information.
  6. All reminders and confirmations will be made by the referral agent directly to the client.
  7. If a client does not appear for the appointment, we will notify the referral agent and
    they can reschedule the client.
